Case histories and discussions are theoretical in nature and for educational purposes only.  Each discussion is unique and not applicable to a specific patient. All recommendations are also theoretical and made by respondents who have not examined any related patient and therefore must not be considered specific advice for management.   Any opinions expressed by a member are their own and private and not the opinion of the AACO. They may not be reproduced or shared outside of the group. No comment in this group is ever made for attribution. No discussion can be reproduced, in part or whole, without the expressed written consent of all involved parties and the AACO Executive Committee.  Remaining a member of the community features of the AACO is de facto acceptance of this policy.  The AACO reserves the right to remove an individual who violates this policy.
